How IMS Treatment Creates Therapeutic Change

When nerves are irritated or compressed (often at the spine), they cause muscles to shorten and
develop sensitive trigger points. These shortened muscles then compress blood vessels, reducing
oxygen supply and creating a pain cycle. IMS needles are inserted directly into these shortened
muscle bands. The mechanical stimulus creates several therapeutic responses:

Local muscle release

The needle insertion causes an immediate local twitch response (LTR), a brief involuntary contraction followed by immediate relaxation. This breaks the pain-spasm cycle and restores normal muscle length.

Neurological reset

The deep mechanical stimulus travels along nerve pathways to the spinal cord, triggering segmental reflexes that normalize muscle tone and reduce nerve sensitization.

Pre-treatment screening:
Before your first IMS treatment session, your physiotherapist will:

  • Confirm you're a safe candidate for treatment
During treatment
IMS sessions typically last 30-45 minutes. Your physiotherapist will:
  • Palpate to identify tight muscle bands
  • Insert sterile, single-use acupuncture needles into affected muscles
  • You'll feel a deep ache or cramping sensation (the local twitch response)
  • Multiple sites may be treated in one session
  • Needles are typically retained for seconds to minutes
IMS treatment frequency:

IMS is cumulative. Most patients require 6-10 sessions for significant improvement. Sessions are typically scheduled weekly or bi-weekly to allow tissue healing between treatments.

Preparing for Your IMS Treatment in Vancouver

24-48 hours before
  • Stay well hydrated
  • Get adequate sleep
  • Continue any prescribed medications
2 hours before:
  • Eat a light snack (don't come on an empty stomach)
  • Avoid caffeine, alcohol, and nicotine
  • Avoid anti-inflammatory medications if possible (they can interfere with the healing response)
What to wear

Loose, easily removable clothing. Tank tops and shorts allow access to common treatment areas (back, shoulders, legs). Your physiotherapist will use appropriate draping for your comfort.

Post-Treatment Care and Recovery

Immediately after treatment

  • You may feel relaxed or slightly fatigued
  • Treated muscles may feel achy, similar to post-workout soreness
  • This is normal and indicates therapeutic response

24-48 hours post-treatment:

  • Muscle soreness typically peaks at 24-36 hours
  • Mild bruising at needle sites is common
  • Some patients feel immediate relief, others notice gradual improvement over several days

Self-care recommendations

  • Stay well hydrated (helps flush metabolic waste)
  • Apply heat to treated areas (warm bath, heating pad)
  • Gentle movement is beneficial (avoid intense exercise for 24 hours)

When to contact your physiotherapist

Frequently Asked Questions About IMS

HOW IT WORKS

IMS treatment uses acupuncture needles inserted into tight muscle bands, or close to the spine, to target compressed nerve roots. Each needle causes a small, therapeutic injury which causes the muscle to cramp onto the needle. This process stimulates spinal cord reactions which relaxes the muscle and returns nerves to regular function. Improved circulation to the area also encourages the healing process.

WHAT TO EXPECT

Once the needle is inserted into the affected muscle, you can expect to feel a twitch or deep ache from the muscle. While this sensation is not painful, it will feel mildly uncomfortable. The achy feeling may last for 24-46 hours. Patients usually see benefits from IMS within 6-10 treatments. Your physiotherapist will determine the appropriate length and timeline of treatment based on your condition.

HOW TO PREPARE

Prior to any recommended treatments your physiotherapist will screen you to ensure you are a safe candidate for dry needling. Before your IMS session you should have a light snack, refrain from any caffeine, alcohol, or nicotine for at least 2 hours, and dress in treatment-appropriate clothing (tank top, shorts, etc.)

POST-SESSION

IMS sessions are cumulative, meaning the more you do it, the better off you’ll be. Time will be allotted between treatments to ensure the tissue heals effectively. If aches or discomfort are felt after a session, try drinking water, soaking in a warm bath, or applying hot compresses to the area. Patients who have experience with acupuncture may find IMS slightly more painful as the needle is inserted into the affected muscle, rather than in specific acupuncture points. Bruising and local tenderness are common side effects of IMS. These effects shouldn’t last more than 24-36 hours.
